MONTHLY MEMBERSHIP:
1. Initial consult
2. Follow up consults
3. Unlimited Acute Care
4. Unlimited email support Tuesday - Friday
5. A typed care plan with every appointment
6. 10% discount on some classes
$299 up front and $97 per month until you cancel
LEVEL TWO: Pay as you go. $299 for the initial consult and $149 for each follow up
LEVEL ONE: Pay as you go. $399 for the initial consult and $199 for each follow up includes email support between follow ups.
FAMILY PLAN: this is an annual family plan where are you paid $2800 upfront and it includes one initial consult for each of up to five immediate family members living in your home. It also includes six follow-up appointments per person per year.
Please enter the name of the person who the appointment is for. Example: If you're the parent scheduling for your child, enter your child's name and not yours.
